Well, it was either that or Zutara, initially, but sometimes I feel like a change of pace is in order, and I've never really heard you talk about Ichiruki.

My own personal fanon dictates that such a thing would have happened somewhere in the SS arc or before the HM arc, one night after they'd snuck out of his house to dispatch a rogue Hollow.

On their way back inside after the fight, which had been bloody and violent, Ichigo reaches his hand to her cheek to wipe away a smear of blood. Before she can even smack his hand away in mild annoyance, she sees the look in his eyes as he does it, and she impulsively leans into his hand - causing the teenaged Ichigo temporary lightheadedness due to blood loss as he sports the hardest and fastest erection of his young life.

Also, did she ever connect her attraction to him with his similarity to Kaien?

Rukia's pretty observant when it comes to Ichigo. I think she would notice it. XD

It's my assumption from what I've seen of canon that while he has a strong similarity to Kaien, physically and personality-wise, she feels more strongly for Ichigo than she ever did her former taichou. I think while they remind her of one another, she keeps them very separate in her head. Not only that, but she'd always thought of Kaien as being untouchable, because of his wife, whom she looked up to - and Ichigo is not. :)
